What is mesotherapy and what types of mesotherapy treatments are available?
Mesotherapy (from Greek mesos, “middle”, and therapy from Greek therapeia, “to treat medically”) is a non-surgical cosmetic aesthetic treatment. Mesotherapy employs multiple ways of injecting plant extracts, vitamins, and other ingredients into the middle dermis.

1.) What is needle-free mesotherapy?
Needle-free mesotherapy is a painless non-bruising technique where natural extracts, vitamins, homeopathic agents and pharmaceuticals, especially hyaluronic acid are passed into the layer of fat & connective tissue under the skin. The presence of hyaluronic acid in epithelial tissue has been shown to promote keratinocyte proliferation and increase the presence of retinoic acid, effecting skin hydration and the ability of cells to hold water. Hyaluronic acid’s interaction with hyaluron receptor cells drives collagen synthesis and normal skin function. Present in the extracellular matrix of basal keratinocytes, hyaluronic acid is critical to the structural integrity of the dermal collagen matrix. These benefits make hyaluronic acid a very effective topical humectant with results sustained over a period of time through an ongoing treatment program.

Hyaluronic acid is also a major component of skin, where it is involved in tissue repair. When skin is excessively exposed to UVB rays, it becomes inflamed (sunburn) and the cells in the dermis stop producing as much hyaluronic acid, and increase the rate of its degradation. Hyaluronic acid degradation products also accumulate in the skin after UV exposure.

2.) What is Micro-needling or Collagen Induction Therapy?
Collagen Induction Therapy (CIT) also known as micro-needling, is a procedure that improves the appearance of wrinkles, acne scars and stretch marks by stimulating your skin’s natural ability to repair itself. It refers to the effect that micro injury to the surface of the skin results in new collagen formation. It is most commonly spoken about in the minimally invasive arena of micro-needling treatments.

Anytime there is injury to the skin, repair occurs through the formation of new collagen.

It is a cosmetic procedure that involves repeatedly puncturing the skin with tiny, sterile needles (micro-needling the skin). It is a technique for which research is ongoing but has been used for a number of skin problems including scarring and acne.

The procedure usually involves a specialized device which can take the form of a micro-needling pen, manual rolling device or an automated stamping device.

The micro-needling pens can control the depth of penetration anywhere from 0.5mm down to 3.5mm in increments of 0.1mm with the most precise devices and by 0.25mm or 0.5mm on others depending on the type of motor.

The skin is topically anaetheticed and then an instrument (whose inspiration may have come from the Marquis de Sade) laden with tiny needles is rolled or gently moved across the skin. The needles penetrate to a depth determined by the therapist by adjusting the pen accordingly corresponding to the depth of the dermis.

In case a needle roller is used, the depth cannot be adjusted and an appropriate roller with a needle depth needed for the specific treatment has to be used over the whole area to be treated.

The penetration of the micro-needles into the dermal layer of the skin creates numerous micro-injuries, stimulating the fibroblast cells to increase collagen production.  Although results are visible within 10 days, the actual result only becomes apparent after approximately 90 days, since it takes around 3 months for the newly produced collagen fibres to mature.  Upon maturation, these fibres shrink slightly, thereby creating the firmer, plumper appearance.

When compared to other anti-ageing treatments such as filler injections, the advantage of the Collagen Induction Pen treatment is, that it activates the skin’s own self-repair mechanism, rather than having a foreign substance injected into the skin. The Micro-needle pen is intended to stimulate the skin for natural collagen induction. It helps with absorption of active ingredients, skin tightening and rejuvenation, acne scar and wound healing, improves wrinkles and fine lines, minimizes pore size, improves stretch marks and alopecia and improves the overall health and thickness of the dermis and epidermis.

What are the side effects?
Depending on the depth of treatment selected, the after-effect is similar to mild-to-moderate sunburn.  With the correct after-care products, the ‘down-time’ is limited to 6-12 hours.  During this time, it is important to avoid the outdoors.  Make-up application is permitted after 12 hours.

3.) What is injectable mesotherapy?
Injectable Mesotherapy is a medical technique whereby active substances are introduced into the skin surface by numerous micro-injections via a syringe, to correct aesthetic disorders such as skin aging, cellulite and alopecia, among others.

Provided it is performed under adequate conditions and with the correct substances, it is a very effective procedure.

Differences between micro-needling (CIT) and injectable mesotherapy is :

a.) when there is a predisposition for post-inflammatory hyper-pigmentation

b.) the patient/client to be treated is under 30 years of age

The method that renders a less aggressive inflammatory response is injectable mesotherapy, as the needle is angled at 40*, which is less aggressive to the skin than micro-needling at 90*.
